At Bravia Dermatology, we take the privacy and security of our patients’ information very seriously.
We are providing this notice to inform you of a recent data security incident involving one of our third-party vendors,
TriZetto Provider Solutions (TPS), a subsidiary of Cognizant.
What Happened?
On or about February 6, 2026, TriZetto reported a security incident to the U.S. Department of Health and Human Services.
TriZetto discovered that an unauthorized party gained access to certain systems on their network server between approximately
November 2024 and January 2026. This incident occurred at the vendor level and
did not involve any direct unauthorized access to Bravia’s internal systems or electronic health records.
What Information Was Involved?
Based on the report provided by TriZetto, the information involved may have included:
- Names and contact information (addresses/phone numbers)
- Dates of birth
- Insurance policy numbers and provider names
- Limited clinical information related to eligibility claims
What Is Being Done?
TriZetto has already sent individual notification letters via U.S. Mail to all affected individuals for whom they had a valid mailing address.
They have also implemented enhanced security protocols, including multi-factor authentication and improved portal monitoring, to prevent a recurrence.
What You Can Do
While there is no evidence that your information has been misused, we recommend the following:
- Enroll in Credit Monitoring: If you received a letter, please follow the instructions to enroll in the complimentary credit monitoring services provided through Kroll.
- Monitor Your Statements: Regularly review your Explanation of Benefits (EOB) and your credit reports for any suspicious activity.
- Stay Vigilant: Be cautious of any unsolicited communications asking for personal or financial information.
