The Patient Portal can be accessed through . Upon your initial visit, you should have received a welcome email providing your user name (usually your email address) and a temporary password. Enter your user name followed by clicking on “forgot password” to have a new password emailed; if you need help obtaining your login credentials, you can always contact us at 419-948-3376. The Patient Portal offers a variety of helpful functions.

Forgot Password

To retrieve your password, you must know your username.  Your username is usually your personal email address you provided at scheduling or during your appointment.

STEP 1. Visit
STEP 2. Enter your email address as Username, Date of Birth, and Last Name.
STEP 3. Check "I'm not a robot" and click on "Send Email"
STEP 4. Check your email for a reset password link.  Please check your junk and spam folders if you did not receive it.  If you are having trouble, please call 419-948-3376 during regular business hours.
